Lung Perfusion CT: Differentiation between Lung Cancer and Progressive Massive Fibrosis
Abstract
dc:descriptionPURPOSE: To evaluate the usefulness of perfusion CT in the differential diagnosis between progressive massive fibrosis and lung cancer. MATERIALS AND METHODS: We retrospectively analyzed the perfusion CT image of 20 lesions in 13 patients with progressive massive fibrosis. Using 16 channel multi-detector CT, a dynamic study was performed by a multiple spiral acquisition technique at 0 (precontrast), 15, 30, 45, 60, 90, 120, 180, and 300 seconds after contrast material injection. Perfusion images were reformatted using these source images. The grades were scored in Volume map and Washout map by visual inspection and their results were compared with those of 25 lesions of lung cancers. RESULTS: Volume map showed either middle or low perfusion at 19 of 20 PMF lesions. Washout map also showed weak or moderate washout pattern in PMF. The kappa values (>0.818) for interreader agreement reached statistical significance (p<0.01). High perfusion on Volume map was 1 case (5%) in PMF group and 23 cases (92%) in lung cancer group. Strong washout on Washout map was 1 case (5%) in PMF group and 17 cases (68%) in lung cancer group. There were significant differences between two groups (χ2=38.248, p<0.01 in Volume map, χ2=36.444, p<0.01 in Washout map). CONCLUSION: Perfusion CT consisting of Volume map and Washout map can be a useful method in the differential diagnosis between PMF and lung cancer.
