Prognosis of Transitional Cell Carcinoma of the Bladder in Patients 40 Years Old or Less : Comparison with Elderly Patients

dc:descriptionPURPOSE: Transitional cell carcinoma (TCC) of the bladder in young patients is rare with 0.4-2% of such tumors presenting in the first 4 decades of life. The natural history and prognosis of TCC of the bladder in young patients remain a matter of debate. This study aimed to evaluate whether TCC of the bladder in young patients is biologically different from elderly patients. MATERIALS AND METHODS: The medical records of 21 patients 40 years old or younger with bladder TCC treated in our institution between July 1995 and May 2005 (group 1) were retrospectively reviewed. For purposes of comparison, the medical records of 94 patients 51 years and older with bladder TCC treated in our institution during same period (group 2) were also reviewed. The clinicopathological variables and survival rates were assessed. RESULTS: Mean age at initial diagnosis was 36.1 years (25-40) and 67.3 years (51-92), and the male-to-female ratio was 2:1 and 3.7:1 in group 1 and 2, respectively. TCC of the bladder was diagnosed between ages 20 and 29 years in 2 patients (9.5%) and between ages 30 and 40 years in 19 (90.5%). Initial stage distribution was similar with 85.7% and 79.8% of patients presenting with superficial TCC in group 1 and 2, respectively. The gross hematuria as a presenting symptom (p=0.943), multiplicity of tumor mass (p=0.298), T stage (p=0.880), N stage (p=0.662), M stage (p=0.635), concomitant CIS (p=0.140), and grade (p=0.065) were not significantly different between the two groups. There was no difference in the cancer-specific survival rate between the two groups (p=0.905). In cases with superficial TCC, no difference was observed in the disease-free survival rate (p=0.821) or progression-free survival rate (p=0.595) between the two groups. CONCLUSIONS: TCC of the bladder in patients 40 years old or less has a similar clinicopathological distribution and disease outcome to those observed in elderly patients. It is suggested that the management and follow-up of TCC of the bladder in young patients should be the same as those for elderly patients.
