Body mass index and response to neoadjuvant chemotherapy in Korean breast cancer

dc:descriptionBACKGROUND AND OBJECTIVES: The relationship of neoadjuvant chemotherapy(NAC) and body mass index (BMI) in patients with breast cancer have been reported in many studies. According to many studies, poor prognosis is associated with obesity in western breast cancer patients, but not in oriental breast cancer patients. We studied the relationship between pathological complete remission, survival and BMI. MATERIALS AND METHODS: This study is a retrospective study through chart review who received NAC at the Ajou University Hospital from 2001 to 2013. Total 217 patients were enrolled, of which 170 patients who performed MRI scan before chemotherapy were analyzed. The BMI of the patients was calculated by age, weight, height. The kind of chemotherapy , tumor size at before and after NAC, operative procedure, postoperative pathological tumor size, lymph node status, nuclear grade , histologic grade , hormone receptor status, epidermal growth factor receptor expression was analyzed. And disease-free survival and overall survival was investigated by recent medical record. According to the WHO classification of BMI, patients divided into four groups less than 18.5, underweight; 18.5 to 24.9, normal weight ; 25 to 29.9, overweight ; 30 or more, obesity. Then, underweight and normal weight were combined to Group A, overweight and obesity were combined to Group B. The definition of pathological complete response (pCR) was confirmed to be the cancer disappeared completely and overall survival, disease-free survival was calculated by Kaplan-Meier method. RESULTS: In this study, 113 (66.5 %) patients in Group A, and 57 (33.5%) patients in Group B. When comparing two groups , factors other than the age not showed statistically significant difference. And clinical complete remission(p = 0.98), pCR (p = 0.63), overall survival (p = 0.64) and disease-free survival (p = 0.10) were not related to BMI. CONCLUSION: Treatment response and survival are not related to BMI in Korean breast cancer patients who receiving NAC. Further additional studies are needed.
