Their expression is inducible by various conditions that present a danger to the survival of cells, such as hyperthermia, hypoxia, heavy metals, ischemia, inflammation etc. Metallothionein is a cysteine-rich metallo-protein. In accordance with heat-shock-proteins metallothionein is inducible by various stress factors and occurs widely spread through prokaryotic and eukaryotic species. In this study the distribution of heat-shock-proteins Hsp27, Hsp32, the Hsp70-family and metallothionein in five groups of non-melanocytic skin tumours and normal skin was examined by immune-histochemistry. The tumour entities were chosen to present increasing levels of malignancy and different aetiologies of keratinocytic aberrations in order to obtain a broad field of data to evaluate proliferation, differentiation and malformation. In spite of a publications on Hsp27, Hsp72 and Hsp73 expression in epidermal tissue there are no comparably detailed studies on heat-shock-proteins in skin tumours. The heat-shock-proteins used in this series showed specific patterns of expression in the examined tumours and skin samples and a deepened and more detailed understanding of their individual functions was retrieved from this study. The constitutive expression of Hsp27 in human keratinocytes was proven in vivo and its expression in perineurium and skeletal muscle of the dermis was demonstrated. In the examined skin tumours we found strong evidence for the significant role of Hsp27 in cell growth and proliferation. An increasing keratinocytic malformation induced by exposition to UV light leads to a reduced or irregular Hsp27 expression. As some of these changes in Hsp27 expression also were noticeable in morphological unchanged skin surrounding the tumours Hsp27 may present an early marker of a disturbed cell differentiation. The expression of the Hsp32 antigene showed no pattern associated with either proliferation or differentiation. Within the Hsp70 family Hsp73 is a reliable marker of differentiation, whose specific staining pattern in normal skin is characteristically altered in all tumours. The morphological characteristics of actinic keratosis are reflected on a molecular level by a reduced and irregular hsp and metallothionein expression. The verrucae vulgares in this study showed no signifikant alteration in their antigene distribution compared to normal skin. In several basaliomas and spinaliomas a focal reactivity of distinct tumour areas was evident, Hsp72 and metallothionein partially highlighted specifically the invading parts of the tumour. In summary this study contributes to a better understanding of the numerous functions of heat-shock-proteins and metallothionein.
