Defining its indications and standardising the methods is a task of high priority in our time. During this prospective study at the Department of Obstetrics and Gynaecology of the RWTH Aachen 446 women with palpatory and/or radiological suspicious breast lesions were examined by mammary ultrasonography, using a 10 MHz-Transducer. The purpose of the present study was to assess the value of ultrasound mammography in differentiating benign from malignant tumours. For ultrasound mammography it could be evaluated a sensitivity of 94,0%, a specificity of 91,4%, a positive predictive value of 95,9% and a negative predictive value of 99,1%. Ten different sonographic criteria were validated separately. The most important signs of malignancy were (in descending order): highly echogenic halo, spikes, jagged contour, jagged or missing dorsal wall, posterior acoustic shadowing and discontinuity of structure. Features of benign disease were: displacement margin, smooth edge, posterior acoustic enhancement, bilateral acoustic shadowing and (among other things) smooth dorsal wall. In addition the influence of tumour size on the prevalence of these signs was evaluated. Furthermore the relation between histological findings and ultrasonographic patterns was analysed.
