This study described clinical and psychosocial aspects of reconstructive and plastic operations of the female breast. 91 reductions, 51 augmentations, 30 corrections of malformations and 20 reconstructions of the breast, perfomed from April 1997 until May 1999, were investigated. All patients were treated in the department of obstetrics and gynecology of the university hospital in Aachen, Germany. The data of complications was comparable with literature. Moreover the reasons of women with macromastie, micromastie or malformation of the breast seeking operative correction were studied. Therefore special questionnaires were developed, focusing on the comparison of the time before and after the operation. The physical problems, but also the emotional pressure were described. It was found that plastic breast surgery is justified under a medical and ethical point of view, but careful and responsible determination of indication is important. Furthermore it was discussed how women with breast reconstruction after mastectomia find back into a normal life.
