Power-spectral values of 24h-holter-monitoring show a large standard deviation up to 100% of the mean values in control groups, which reduces reliability and the predictive value of this method. The analysis of circle maps are a new method for analyzing respiratory sinus arrhythmia based on phase dynamics. Differences in circle maps are quantified by the Kullback information gain, a statistical measure used in information theory. The more two circle maps differ, the more new information is gained, decreased Kullback information gain showing decreased difference between circle maps. We supposed patients after AV-node-modification (AVNM) to present a lower autonomic innervation level in 24h-holter-monitoring and Kullback information gain. 18 young (25 ± 4 years, 10m, 8f) and 14 old (58 ± 10 years, 7m, 7f) healthy probands underwent 24h-holter-monitoring and a paced breathing protocol. 16 consecutive patients (47 ± 19 years, 4m, 12f) underwent 24h-holter-monitoring and the same paced breathing protocol before and after AVNM. In H, ultra-low (ULF), very-low (VLF), low (LF) and high frequncy (HF) bands as well as LF/HF ratio were analyzed. Circle maps were obtained and differences in cardiac phase-dynamics were analyzed with the Kullback information. Power-spectra of frequency bands are decreased in older probands compared to young probands of the control group. A decrease of power-spectra initially after AVNM and normalisation within 3 month can be observed in patients before and after AVNM. There is no difference between intraindividual Kullback information of patient- and control group. Autonomic innervation is decreased in older probands compared to young probands of the control group. Frequency-domain parameters of 24h-holter-monitoring, but not Kullback information gain are altered by AVNM.
