Studie zur Bedeutung der In-vitro-Blutungszeitmessung mit dem PFA 100 für die Vorhersage peri- und postoperativer Blutungskomplikationen in der Gynäkologie

Bleeding during or after operations is one of the most often complications in surgery. Until today there is still no valid test to prove a patient's bleeding habit preoperatively. In this study we proved the advantage of the new platelet function analyzer 100 (PFA 100™) compared to the standard coagulation tests and a patients history concerning coagulation. In addition to this we proved the thrombocyte function during operations. 116 patients who underwent gynaecological surgery were included. All parameters were proved according to the dimension (small/ big) and mode of operation (laparoscopic/ non laparoscopic). Our data shows that non of the proved tests was able to make a valid prediction in reference to pre- or postoperative bleeding complications in gynaecological surgery. In addition to this it was not possible to improve the predictiv value of bleeding complications with the PFA 100™ compared to the standard coagulation tests. Our results can not justify the performance of the in vitro bleeding time in patients who undergo surgery in gynaecology, since there is no evidence that this new and expensive test is a more sensitive parameter to predict bleeding complications compared to the hitherto existing preoperative standard program.
