Psychopathologische Befunde bei Patienten einer umweltmedizinischen Ambulanz

Introduction: The aetiology of illnesses associated with the environment is unknown. A certain sensibility to environmental pollutants and possibly a psychiatric genesis of illness in many patient histories is being discussed. Methods: All patients (n=50) who consulted the outpatients´ unit of environmental medicine at the university hospital of Aachen between the 1st of January and the 31st of December 1997, all assumed a harmful influence by environmental pollutants on their health, were tested. Beside other interdisciplinary examination and testing, a psychiatric interview had taken place, a diagnosis compliant with the ICD-10 had been given and the questionnaires BDI, STAI-X1, STAI-X2, SCL-90-R, SOMS-7T and SOMS-2J had been filled out. Results: 76% of the examined patients received a psychiatric diagnosis. In 64% of the cases the diagnosis was considered to be the primary cause of illness. The most common psychiatric diagnoses given were neurotic, stress-related and somatoform disorders (F40-49) at 64%, followed by shizophreniform and delusion disorders (F20-29) at 14% and affective disorders (F30-39) at 12%. Conclusion: The main fraction of diagnoses considered to be the primary cause of illness were psychiatric diagnoses. As long as there is a disagreement about the definition, aetiology, pathogenesis, diagnosis, therapy and prognosis, only interdisciplinary diagnosis and examination can help the patients. Such concepts should be the basis for further scientific investigation. Furthermore, there is a need of a standarized procedure in the diagnosis of these illnesses to make studies comparable.
