Inzidenz der katheterassoziierten Infektionen auf einer kardiologisch-pulmonologischen Intensivstation : Risikofaktoren und Bedeutung klinischer und laborchemischer Parameter

dc:description309 patients received 572 central vascular catheters during the study period. The mean duration of catheterization was 4,9Þ2,8 days. 59 catheters (10%) were removed because of a suspected catheter-related infection. The catheter infection rate was 4% (8,1/1000 catheter days), the catheter bacteraemia rate 1,7% (3,5/1000 catheter days). The CRP-level did not help to diagnose catheter-related infection. A leucocyte count >16,0x103/ml correlated significantly with a negative catheter tip culture. A frequently falling temperature after removal of catheters with negative cultures was an indicator for the impact of non-infectious reasons of catheter-related fever. An ICU length of stay >14 days and the diagnosis 'infection' on admission were patient-related, a repeated catheterization and a duration of catheterization >5 days catheter-related risk factors. The infecion rate per catheter day remained unchanged with prolonged duration of catheterization but raised with repeated catheterization. A higher infecion rate was observed for quadruple-lumen compared to single-to triple-lumen catheters and during parenteral nutrition. Catheters inserted in the second study period (less experienced physicians and nurses) had a higher risk of infection.
