Tensiometrie bei der Leistenhernienreparation nach Shouldice
Abstract
dc:descriptionOpen or laparoscopic "tension-free" hernioplasties and their advantages in contrast to conventional Shouldice repair have been the topic of many publications. Their benefits are bought by permanent implantation of a mesh, which is not undisputed among hernia repair specialists. A study was designed to investigate the Shouldice repair, which works without foreign materials, but is said to produce a high level of tensile strength (TS). Therefore force transducers including strain gauges for the qualitative analysis of TS were fixed to the inguinal ligament and the lateral edge of the rectus sheath. Measuring the distance between the force transducers and the TS during adaptation and stretch of the branches of the instrument it was possible to analyse the elasticity of the abdominal wall. Additionally the influence of rising intraabdominal pressure (IAP) on TS by coughing or abdominal press was measured. Analysing elasticity an elasticity of 3.13 ± 0.29 N/cm during adaptation and 6.50 ± 0.66 N/cm during extension was found. Mean wideness of the hernial gap was 1.19 ± 0.17 cm. During coughing an increase of tensile strength of 2.98 ± 0.38 N, during abdominal press an increase of 0.95 ± 0.11 N was quantified. The low level of TS induced during the repair and the small effect of increased IAP on anatomical structures represent no danger for an appropriately performed reparation.
