Mikrochirurgische Therapie zervikaler Bandscheibenvorfälle : Nachuntersuchung von 140 operierten Patienten

Between 1993 and 1995, 176 patients underwent surgery for cervical degenerative disc disease. 105 (75%) patients with radiculopathy, 14 (10%) with myelopathy and 21 (15%) patients with a combined symptomatology were followed up one to four years postoperatively. A soft disc lesion was found in 55 (39%) patients, a hard disc lesion in 42 (30%) patients. 43 (31%) showed a combined hard-/soft disc symptomatology. An excellent or good long-term result was achieved in 91% of patients with radiculopathy and in 86% of those with myelopathy. The outcome was best in cases with soft disc lesions (96% good or excellent results), whereas patients with hard disc lesions achieved good or excellent results in only 79% of cases. An acute onset and a short duration of symptoms proved to be a good prognostic value for favorable postoperative outcome. The overall outcome of the anterior discectomy with polymethyl-methacrylate (PMMA) fusion was equivalent to the posterior interlaminar sequesterectomy.
