Lebensqualität von Patienten mit koronarer Herzerkrankung in hausärztlicher Betreuung

This paper discusses the disease-specific quality of life of patients with coronary heart diseases in familiy pracitioners care. 107 Patient received a quality of life Questionnaire. Patients were enrolled from a cardiac catheterization laboratory and they where in continuous care by 30 general practitioners in the region of Aachen. The german version of the Seattle Angina Questionnaire was used to evaluate health related quality of life. 58 (55 %) Questionnaires were analyzed. Differences in physical limitation were dependent on gender and age of patients. Women felt stronger physical limitations, even when their symptoms were rare in the last weeks. 70 –79 year old patients were less affected by physical limitations than younger patients, which might be explained by a so called “Kompetenzmodell”. Higher satisfaction of elderly patients with their family practitionners care could be related to higher disease-specific quality of life. In terms of cardiac risc factors diabetes mellitus showed strong negative influence on health-related quality of life. Hypertonus, Nicotin and high Cholesterin showed only minimal influence in certain scales, but overweight patients gave higher points for disease-specific quality of life, which might be important for the relationship between patient and family practitioner. Overall patients showed a very high satisfaction concerning their therapy and explanations, which can be seen as a benefit of the high engagement of the general practitioners. Non the less patients at the age of 50-59 years were notably less satisfied with their family practitioner and much more affected by regulary intake of tabletts than patients at the age of 70-79 years.
