Untersuchung zum Zusammenhang zwischen Zahnbehandlungsangst und kardiovaskulären Parametern bei Betrachtung des affektiven, kognitiven und somatischen Angsterlebens

The presented study explored at 100 patients from 3 dental offices the correlation between dental anxiety and cardiovascular response considerating as well anxiety on an affective, cognitive and somatic level as the coping of fear.Dental anxiety was recorded using the ‘Aachener Zahnbehandlungsangst-Inventar’ (AZI) which was constructed from an interactive approach to anxiety. The design enables the recording of dental anxiety, a differantiated view on the affective, cognitive and somatic aspect of dental anxiety as well as the developping of dental anxiety in seven chronological situations within a dental treatment. Cardiovascular parameters were measured three times during the stay in the dental office.Results showed an increasing dental anxiety on approaching the dental treatment itsself with a maximum of fear recorded before the treatment. An explanation was found in coping which took place before the dental treatment. Further on dental anxiety was recorded on a different extent for the affective, cognitive and somatic level. The lowest scale was found for the somatic level.Analysis of the cardiovascular parameters showed a peak in diastolic blood pressure and heart rate after the dental treatment. A significant relation to dental anxiety was not found. Patients with a higher anxietyon the somatic level showed increased but non-significant cardiovascular responses.Results for the coping of fear showed patients with a high AZI-score prefering vigilant, patients with low AZI-score an acoidant coping style.Coping of fear and cardiovascular parameter did not correlate. The coping could not explain the corellation between dental anxiety and cardiovascular response.
