Langzeitergebnisse nach operativer Behandlung der Colondivertikulitis

In a follow up study we investigated the long term results after the surgical treatment of 179 patients (90 men, 89 women, age median,range: 59, 25-86 y.), who were operated in the University Clinic in Aachen from Jan. 1990 til Dec. 1995 because of colonic diverticulitis. 2 patients were excluded from the study because of early postoperative death and 5 patients, who lived in foreign countries. All other patients got a questionnaire and a telephone interview was performed. If the patient could not be reached, his family doctor or relatives (mainly in the pat. who died during the follow up) were contacted. 102 patients answered the questionnaire or the telephone interview, in 27 patients the informations were got from the family doctor and in 12 patients from the relatives. In 31 patients we could get no information at all, neither from the patients themselves nor from relatives or the family doctor. During the follow up (median, range: 92 month, 65-136 m.) 21 patients died. In 6 cases the cause of death was not exactly known, but in no case there were any signs of a relation between the death and the diverticular disease. 13 patients developed a new diverticulitis, all 13 were treated conservatively. 6 patients developed a severe colonic bleeding, which could be treated conservatively in all cases. 23 patients complained about severe symptoms due to the abdominal scar or adhesions. 13 of them developed an abdominal hernia, which was operated in 5 cases. 3 patients were operated because of an ileus due to adhesions and one patient needed an operation because of the rupture of the colonic anastomosis during an endoscopic dilatation. In 10 patients further abdominal operations (4x gall bladder excision, 3x gyn. operations, 2x urlog. operations , 1x inguinal hernia) were performed without any relation to the primary diverticular operation or recurrent diverticular disease. Conclusions: In the long term follow up after surgical treatment of colonic diverticulitis only very few patients develop a recurrent diverticulitis or colonic bleeding, which can be treated conservatively in most cases. A comparatively high number of patients however complain of severe symptoms due to the abdominal scar or need a reoperation because of abdominal hernia or ileus. We hope, that the laparoscopic approach, which was only performed in 6 patients of our study , but which is common today in the operative treatment of diverticular disease, will minimize these problems.
