Immunhistochemische Analyse des p16-Proteins bei Patientinnen mit primärem Mammakarzinom und seine prognostische Relevanz

dc:descriptionIn order to study the role of the p16 tumor suppressor, an important protein in the transition of cells into the G1phase of the cell cycle, in breast cancer, we analysed the prevalence of p16 protein expression in 103 breast cancer samples over a retrospective period of 12 years. P16 protein expression was evaluated immunohistochemically on paraffin sections using monoclonal antibodies and the results were compared with established prognostic parameters. In the univariate survival analysis we first demonstrated the significant association of the patients survival time with histologic staging (pTMN =0,0005, pN = 0,0005, pT = 0,0141, pM = 0,0081) and with histologic grade (p = 0,003). Studying the validity of the estrogen and progesterone receptor status, we only found a statistical significance with the progesterone receptor status, which is absolutely in agreement with the consent of St. Gallen 1998 (p = 0,0142). The p16 nuclear reactivity however did not show a significant correlation neighter with the patient survival time, neighter with the histopathological parameter. Surprisingly the cytoplasmatic accumulation of the p16 protein turned out to be a indicator of unfavourable prognosis with shorter survival time (p = 0,0013). Further cytoplasmatic overexpression was significantly correlated with high grading ( p = 0,0067), including rapid mitosis (p = 0,0013) and strong nuclear polymorphy (p = 0,0259). The clearest result was found in the relationship between the p16 cytoplasmatic expression and the hormone receptor status: a highly significant correlation with negative estrogen and progesterone receptor status. (p = 0,0001 and p < 0,0001). In a following Cox multivariate analysis the established histological parameter like nodalstatus, progesterone receptor status and tumor size were confirmed as independant predictor of patient outcome, unlike p16. We conclude that p16 cytoplasmatic reactivity is indicative of more undifferentiated malignant phenotype in mammary carcinomas with a poor prognosis but however p16 could not reach the stage of an independant prognostic marker.
