Because it is known that changes of the extra cellular matrix appear in hernia patients (alterations of collagen type -I/-III ratio, expression of MMP’s), it was the frame of this study to examine, which changes could be observed during mesh implantation in the extra cellular matrix without patient depending influences in a standardised animal study. Meshes of different materials, poresize und weight were examined in respect of their integration: Mersilene, a low-weight polyethylene small pore mesh; Pronova, a low-weight polyvinylidenfluorid small pore mesh; Prolene, a heavy-weight polypropylene small pore mesh; Softpro, a low-weight polypropylene large pore mesh; Vypro and Vypro II, as low-weight large pores meshes of a combination of polypropylene und absorbable polyglactin 910, as well a low-weight polyglactin 910 small pore vicrylmesh. The diameter of the resulting foreign body granulomas as a quantitative feature of the inflammatory reaction and the filament distance, on which a connective scar structure (scarred bridging of the pore) was analysed. Because in case of incisional hernia a disturbance of the collagenmetabolism is discussed, it was also examined, which influences the different meshes on the collagenases MMP-1 and -13, their inhibitor TIMP-1, as well as the ratio of collagen Type-I to collagen Type-III have. It could be shown, that the size of the developing granuloma and so the extent of foreign-body reactions are dependent on implantation time as well as on the quantity of material and structure of the mesh. Here it was shown in general, decreasing foreign body reaction with the lowest expression for low-weight and large pores meshes during implantation time. Consecutively it was found that, in these meshes there was no formation of a complete perifilamentar fibrosis and so integration of the implant in a scar framework. The analysis of the collagen Type -I/-III ratio showed, that the ratio increased between the 3rd the 21st and the 90th day in the sense of a maturing of the scar. Qualitative differences between the meshes were not established. In contrast it could be proved in comparison to normal skin- respectively scar tissue decreased ratio as a sign of persisting chronic remodelling to the end of the examination period. The expression of MMP-1 and -13 and their inhibitor TIMP-1 could be shown immunohistochemical in all foreign body granulomas. At the same time, the expression seems diffuse, without significant relation to the kind of mesh material and the implantation time.
