It has been demonstrated in different clinical studies that endovascular irradiation can reduce restenosis after stenting or angioplasty. The aim of the study was to examine the effects of radioactive self-expanding 188-rhenium stents on neointimal formation as well as on the percent area stenosis and the histological parameter endothelialization, oedema, apoptosis, inflammation, thrombosis and the grade of smooth muscle cells in the neointima. We implantated thirty stents, 15 of which were radioactive (mean activity 21,02 MBq) and 15 of which were non-radioactive (control stent), into the external iliac arteries of 15 sheep. Furthermore we investigated the influence of an additional traumatization of the vessel wall. The study showed a reduction of the smooth muscle cells in the neointima by the 188 rhenium stents. However there was no positive effect on the neointima formation and the percent area stenosis. In further studies it must be examined over a longer period of time if the inhibition of smooth muscle cells results in a reduction of neointima formation and percent area stenosis. In the group with the radioactive stent and an additional traumatization we could demonstrate a reduction of neointimal proliferation and percent area stenosis compared to the animals with radioactive stents and no traumatization. Our results suggest a benefit of irradiation in tissue with an additional traumatization. There was no difference in the endothelialization between the radioactive and the control stents. In both groups were only few endothelial cells, but no evidence of thrombosis. Compared to the control stents the number of apoptotic cells were reduced in the neointima of the radioactive stents. An additional traumatization resulted in less apoptotic cells in the radioactive as well as in the control group. With regard to oedema and inflammation there was no difference between the animals with the radioactive stents and the control stents.
