Postendodontischer Aufbau von Zähnen mit FRC-Stiften : Ergebnisse einer prospektiven klinischen Studie

dc:descriptionTwo groups of glass fibre-reinforced post systems for the restoration of endodontically treated teeth (FRC Postec®, IvoclarVivadent, FL-Schaan und DT Post®, VDW/RTD, F-Saint-Egrève) were compared in a prospective clinical trial with at the Medical Center of the RWTH Aachen University. Between October 2001 and August 2006 a total of 168 posts were adhesively inserted by nine experienced dentists in 150 teeth following a fixed protocol. From a total number of 100 patients, 39 received 41 posts in 39 teeth in the FRC Postec® group and 54 received 105 posts in 91 teeth in the DT Light Post® Group. Associated with two posts only a composite reconstruction was performed, without any further indirect restoration. In direct procedures, 138 teeth were restored with posts, composite cores and crowns. In indirect procedures 8 teeth were restored with a post, a core reconstruction fabricated of an experimental glass-ceramic. The Final restoration was performed with all-ceramic crowns. As dentin adhesive a multiple step system (Syntac system was utilized with 130 posts and for the other 16 posts, Excite® DSC or Panavia® ED Primer was applied to dentin. With a modified Rocatec® method and additional silanization 126 post surfaces were pre-treated and subsequently inserted with a dual-curing resin cement, Variolink® II. Twenty posts were corundblasted, silanized and placed with a dual-curing resin cement, Panavia® F 2.0. For the new industrially preconditioned DT Light Post® SL only Variolink® II was used as resin cement. Finally, the preparation for an all-ceramic restoration (partial crowns, veneers, crowns and fixed bridges) was performed, when possible in the same appointment. The mean observation period for the FRC Postec® group was 49 months to 65 months and for the DT Light Post® group 20 month to 45 months. Major complications, like root or post fracture, were not observed until the last recall appointment (01.04.2007). One minor complication was detected in the form of a marginal defect in a maxillary incisor, restored only with a complete composite reconstruction. This tooth did not receive a definitive indirect restoration and this complication was recorded as marginal discoloration. The clinical results of fibre-reinforced composite post systems presented in the literature are encouraging. The results of the present in-vivo study showed, that the success rate of glass-fiber reinforced posts was of almost 100% after 3.4 years of clinical service. For both groups, direct or indirect procedure therefore it can be concluded, that the postendodontical restoration with glass fiber-reinforced posts employed in the present study can provide satisfying medium-term clinical success rates.
