Raumfordernde A.cerebi-media-Infarkte : ist die Mittellinienverlagerung ein prognostisches Kriterium?

dc:descriptionIn this study the illnesses of 46 patients (30 men, 16 women) admitted to and treated in the Neurological Intensive Care Unit of RWTH Aachen University after a complete media infarction in the period May 1992 to March 2002 were analysed retrospectively. A total of 3042 patients were admitted to the intensive care unit in this period. A mid-line displacement at the height of the Septum pellucidum as a result of a space-occupying process was observed in all 46 patients by means of CCT examinations. The objective was to analyse on the basis of the data whether there is a correlation between the extent of the mid-line displacement in the brain and the chances of survival of the patients. The extent of the mid-line displacement at the height of the Septum pellucidum was measured linearly and converted to original size. The results were compared with the results of other authors using a chi test. For statistical purposes (chi test) a mid-line displacement of 10 mm was fixed as limit in the horizontal plane for the survivors and deceased of the infarct. The patient data were (survivors or deceased) entered in a four-cell table. Using a chi test the correlation between the extent of the mid-line displacement at the height of the Septum pellucidum and the patient outcome was examined. The chi test resulted in a p-value of p = 0.1515. We suspect there is no significant correlation between mid-line displacement in the brain and patient mortality. Although CCT examination of patients with severe neurological defects following a stroke results in high diagnostic accuracy, the extent of the mid-line displacement in the CCT does not deliver any safe prognoses. The clinical significance of a mid-line displacement has been discussed contradictorily in the literature. The question whether an isolated observation of mid-line displacement after an infarct may be seen as a safe prognostic criterion remains open.
