It still remains unclear if these EPCs are mobilized specifically and if they present the same functional potency as those of patients with chronic coronary heart disease. The aim of this investigation was therefore to describe the amount and the functionality of EPCs in an acute myocardial infarction and to identify clinical, laboratory and angiographic correlatives of an increased EPC amount. 40 patients with an acute ST- elevated infarction were included and compared to a control group consisting of 40 patients with a stable coronary heart disease. All patients underwent a successful PCA and a stent implantation. Both groups were matched according to age, sex and statin treatment as determinants of physiological EPC mobilization. Results showed that a STEMI lead to a homogeneous mobilization of mononuclear cells and EPCs and that acute mobilized EPCs of patients with STEMI are less functional potent than those from patients with stable chronic heart disease. Furthermore it could be demonstrated that an assumed association between the amount of EPC and the grade of the myocardial reperfusion in the territory of the successful recanalized, infarction dependent artery is not existent. Summing up it can be said that the unspecific physiological mobilization of EPCs in context of the acute myocardial infarction does not support the concept of the specific EPC treatment.
