Vergleich zweier Messverfahren zur Überprüfung der Implantatgenauigkeit in der Navigationschirurgie : Computertomographie (CT) vs. Koordinatenmessgerät (KMG) ; eine In-vitro-Studie

dc:descriptionDue to the demographic evolution observed in the industrial nations, there is a growth of dental implantation of the aging population. Precise operative planning, realization, success and survival rate of the insert implants trough computer aided surgery (CAS-) techniques places strict requirements on implant positioning to reach a high medical benefit out of function, esthetic and prosthetic view. The aim of the following in vitro study is to evaluate the accuracy of conventional measurements based on postoperative computer tomography (CT) data, as described by various authors, and determining its suitability for dental implant positioning. To carry out this investigation, 40 Brånemark MK III implants were inserted into 10 human cadaver mandibles. After insertion through intraoperative navigation using optical tracking, a measurement was taken first with the conventional CT-measurement technique and subsequently with a coordinate measuring machine (CMM). The CMM employed provides sufficient single point articulation performance, allowing it to be used as a reference system in evaluating the acquired CT Data. The determined postoperative results of both measurement systems were mapped into a uniform coordinate system and compared with the preoperative planning data. For interpretation of the measurement results, a connected t-test (significance level a = 0.05) was performed based on the euclidic distances in the three-dimensional space. The determined p-value < 0.0001 indicates a significant difference in implant position caused by inaccurate CT-measurement when compared to the coordinate measuring machine. The conventional measurement based on CT-data does not provide the adequate positioning accuracy required for dental implants, due to the lack of sufficient spatial resolution. Therefore, based on the results of this study, the CT measurement technique should not be used for postoperative implant position measurements. These results reaffirm the requirement that a measurement system needs to satisfy the "golden rule", i.e. a relation of 1:10 between object and resolution, for the measurement system to be usable. Until now there exist only rudimentary approaches for the realization of a measurement system, which guarantee an exact, beam-free measurement of postoperative implant position. Prospective research should develop a relevant, clinical measurement system, which fulfils these conditions.
