Our aim was to investigate the signal transmission leading to intramyocardial expression of TNFalpha and COX-2 using the pig model. Another focal point was further research concerning the effects of core temperature during the cardiopulmonary bypass on the expression of TNFalpha and COX-2 in the myocardium. 12 pigs were involved in the study. Each 6 pigs were randomised to the group operated under moderate hypothermia (28°C) or to the group operated under normothermia (37°C), respectively. Altogether, we recruited 21 pigs for the investigation of hemodynamics, operation parameters and temperature: 11 pigs in the hypothermic group and 10 pigs in the normothermic group. Myocardial tissue samples were acquired before, during and 6 hours after the cardiopulmonary bypass. TNFalpha-mRNA was detected by means of the competitive transcriptase polymerase chain reaction. The protein levels of TNFalpha, iNOS and COX-2 were evaluated using western blot during obtaining the activation of NFkB and AP-1 by means of EMSA with supershift and/or western blot. Animals operated under hypothermic conditions showed lower TNFalpha- and COX-2-expressions during and after cardiac surgery, but no changing in iNOS-synthesis. This was associated with a lower activation of p38-MAP-kinase and AP-1 in hypothermic treated animals. NFkB acts completely different by showing no distinction between the both testing groups. The results of this investigation show that by means of moderate hypothermia during the cardiopulmonary bypass the intramyocardial expression of TNFalpha and consecutively the expression of COX-2 induced by TNFalpha can be reduced by activating the p38-MAP-kinase-AP-1-pathway, but not by inhibiting NFkB.","abstract_html":"Cardiac surgery by use of cardiopulmonary bypass goes along with extensive systemic inflammatory response.
