In these patients the right ventricle is often exposed to a major chronic pressure overload. Even though this situation regularly leads to a compensatory hypertrophy, the chronic overload may result in right heart failure. The major reason for the deterioration of cardial function seems to be triggered by the myocardial remodeling processes due to chronic pressure overload. A better understanding of these processes seems to be necessary to develop preventing strategies and to find the best point in time for intervention. The aim of our study was to establish a standardized model of a moderate pulmonary stenosis in neonate lambs (n=17) in order to estimate the effects of right ventricular pressure overload on the systemic and intramyocardial production of the pro-inflammatory cytokines TNF-alpha, IL-1beta, IL-6, IL-8 and the anti-inflammatory cytokine IL-10. In this model the newborn lambs were exposed to a pulmonary banding for a time period of three months. By measuring the hemodynamic parameters after one, four and twelve weeks, we achieved a good overview over the development of the right ventricular pressure overload. The RTQ-PCR analysis of the biopsies, which were taken at the third catheter intervention after twelve weeks, showed a slightly increased intramyocardial expression of all five cytokines (TNF-alpha, IL-1beta, IL-6, IL-8 and IL-10) in the pressure overloaded lambs in comparison to the control group. The difference in this expression was not significant between both groups. The analysis of the blood samples did not show a significant difference in the systemic expression of cytokines between both groups as well. However, the animals pertaining to the banding group revealed constantly ascending blood levels of all five cytokines for the testing period of three months. Those ascending levels showed the same gradient and correlated with the continuously growing right ventricular pressure. This coherence implies a slight, but still measurable effect of the chronic pressure overload due to a moderate pulmonary stenosis on the systemic and intramyocardial expression of pro- and anti-inflammatory cytokines as well as the connected risk of myocardial remodeling.
