The basic factor for the anastomotic healing process is an undisturbed microcirculation. The aim of this injection study was to analyse the arrangement of arteries in the mesentery and of the arteriae rectae in man and to compare it with the anatomy of pigs. Arterial arcades, a characteristic of the mesentery in man, are absent in pigs. Instead, a parallel to the intestine running band of numerous, anastomosing arteries gives rise to many bundles of arteries. The significance of this arrangement of vessels is not known. Thus the transferability to man of the results of studies in surgical research, where pigs are used as models, has to be verified. Additionally, we studied the influence of an enterotomie versus an anastomosis on the vascularization of intestine. After antimesenterial enterotomie there was a smaller area without vessels than after end-to-end-anastomosis. With our results the high rate of complication after enterotomie as often described in literature cannot be explained by the anatomical circumstances.","abstract_html":"Anastomotic leakage still forms a serious complication in gastrointestinal surgery associated with high mortality.
