Vergleich der Einflußgrößen unterschiedlicher Prognosegruppen nach Subarachnoidalblutung

dc:descriptionIn this retrospective study the quality of life and the outcome of patients after spontaneus non-traumatic subarachnoid hemorrhage (SAH) was assessed. The follow-up was performed in two groups: 172 patients after aneurysmal SAH and 42 patients after SAH without a proven source of bleeding. Clinical and neuroradiological variables and particularly the results of daily Transcranial Doppler sonography (TCD) were considered. For the evaluation of the neurological outcome the Glasgow Outcome Scale (GOS) was used. The measurement of health-related quality of life was the Aachen Life Quality Inventory (ALQI). This questionnaire was answered by the patients themselves and by their life-companions. The initial clinical state according to the Hunt and Hess grade was the most important causal factor for quality of life independant of the etiology of SAH. The patients age proved to be the second important predictor for later quality of life in both SAH groups. Elevated and prolonged mean blood flow velocities with symptomatic and/ or asymptomatic vasospasm could be demonstrated as significant and suitable parameters for predicting the outcome of patients and also the quality of life after aneurysmal SAH as well as after SAH of unknown cause.
