Basic fibroblast growth factor (100µg), an extremely potent mitogen or bone morphogenetic protein (100µg), whose osteo-inductive effects are well-known, and 1 cm³ hyaluronic acid (Hyalart 10 mg/ml), a component of the extracellular matrix, were combined to form a viscous gel. This composite was developed so that it could be introduced into the most diverse cases of bone defects by means percutaneous injection. In an experimental fracture model a defect of a critical dimensions (diameter 9,4mm, depth 10mm) was performed into the distal femur epiphysis of 25 sheep. In 15 animals the defect was filled either with hyaluronic acid (Ha), Ha+bmp2 or with Ha+bfgf; as a control group the other animals were either treated with autologous bone or the defect was left empty. X-ray and fluorescent photos (12 weeks after the operation) already showed macroscopically clearly that no new bone material had formed. In the control group, however, the results were analysed histo-morphometrically because precisely in the spongiosa plastic group a considerable quantity of bone material had formed. The mean value in the spongiosa plastic cases lay by an increase of 19,97% of new bone material, whereby this reading combines the quantity of implanted bone and newly-formed bone in one figure. In the cases of the empty defects there was an increase of 5,11%. The study shows that composites of Ha+bmp2 or Ha+bfgf are not useful as bone replacment materials. The positive results of the spongiosa plastics confirm the present situation in the treatment of bone defects.
