Abstract

dc:description

The aim of research work: to investigate the relationship between intensity of fatigue and prevalence of urinary incontinence forms of the nursing staff (nurses and nursing assistants) working in multifunctional hospital. Tasks of work: 1. To investigate the demographic, social, and lifestyle data of the nursing staff (women who work in a multifunctional hospital); 2. To investigate the prevalence of urinary incontinence and the types of it, as well as the scale of fatigue; 3. To analyze and evaluate the relationship between demographic and social factors of urinary incontinence disorder types; 4. To investigate the relationship between the urinary incontinence and fatigue scale. Materials and methods. The study involved 100 nursing (female) employees (78 of them worked as nurses, 22 – as their assistants). Anonymous survey was conducted in February and March, 2016 at Vilnius University Hospital Santariškės Clinics. Some specific questionnaires for sociodemographic and lifestyle data collection were designed : “Urinary and genital diseases caused by the negative effect of inventory description in a short form "- (UDI-6) in order to determine the types of the urinary incontinence; "Overactive bladder symptom score” (OABSS, J.G. Blaivas et al., 2007) in order to assess the overactive bladder symptoms and their intensity, frequency and the impact for lifestyle; "The Multidimensional Fatique Invetory” (MFI-20, validated by A. Stankus, 2002) for an objective assessment of the overall fatigue and scales - physical, mental, reduced activity as well as motivation. Data analysis was performed using SPSS 17.0 for Windows and Excel 2010 programmes. Results. Half of the respondents who have participated in the study were older than 41 years. 15% of the women worked in an intensive care unit, 32% – in a surgery and 62% in regular care units. 60% of the subjects worked very hard in shifts. 35% of the women gave birth 3 times or more and were more susceptible to the urinary tract infections accordingly. We found that the most common type of the urinary incontinence was stress incontinence, which type (p<0.05) correlated significantly with the older age and heavier physical work. The most notable of all the overactive bladder symptoms was uncontrollable urge to urinate, which causes the interruption of all the activities and rush to the bathroom, the lack of bladder control. Stress incontinence was significantly (p<0.05) and strongly related to the total (r=0.82), fatigue (r=0.73), and decreased activity (r=0.52). Quite a weak but rather meaningful effect on the incontinence was a mental fatigue and reduced motivation. Conclusions. 1. The nurses and their assistants with urinary incontinence were mainly working-age women (between 41 to 56 years), most of them (60%) worked in full time shifts and heavy physical work (a lot of walking, lifting, and carrying) and most of them have worked in therapeutic units (62%). The most common illness was the inflammation of the bladder (68%). 2. The 3 main types of urinary incontinence were discovered, where stress incontinence was the most significant. Overactive bladder symptoms fluctuated between weak to moderate and the urinary control was an average or bad. General and physical fatigue has obviously dominated in daily activities. 3. The significant relationship was identified between the stress incontinence and the age and heavy work. 4. The significant and strong correlation between the urinary incontinence and general fatigue and its scales (physical tiredness and reduced activity) was determined. Weak motivation as an expression of the fatigue and mental fatigue interacted less with the urinary incontinence.
