<p>Homecare Portacath Protocol in IVIg Administration</p> <p>The objective for this CNL internship project was to close a practice gap that was discovered while evaluating my microsystem for system improvements. My microsystem is a medium sized Home Health Agency with a specialty of in home infusion therapy. While evaluating protocols for immunoglobulin (Ig) infusion I found that there was no protocol for the administration of Ig by portacath. After consulting with the Director of Nursing (DON) and my colleagues on the Infusion Team, it was agreed that a current evidence based protocol was needed to close the practice gap. The methods used in the evaluation and implementation of this project involved a root cause analysis of the problem and was tested with a plan-do-study-act (PDSA). An evidence-based protocol for IVIg by portacath administration was created, by utilizing the Infusion Nursing Society [INS (2011/2016)], standards of care as the guiding reference. Training was initiated using Bandura’s four- step model as a foundation for the implementation of my project (as cited in Bastable, 2014). After the completion of the protocol and training, a Likert survey was used as an evaluation tool. The project and newly created protocol have been implemented as the standard of care for IVIg portacath administration; with an annual review to insure the maintenance of current evidence based standards.</p>
