Optimizing Electronic Healthcare Records and Improving Process in the Healthcare Clinic

<p>Abstract</p> <p>The purpose of this project is to examine the reasons for low compliance in entering data into the electronic healthcare record (EHR) and implement a change to improve the process. The project consists of generating data and reviewing charts to determine the reasons data on homelessness and income status are not entered in the EHR. If data is not entered, this may lead to a threat to the quality of care provided by the clinic and a potential loss of status as a Federally Qualified Healthcare Center and recognition as a Patient-Centered Medical Home. The baseline data shows that staff only enters 1.5% of the income and 65% of the homeless status. One of the main issues of low compliance is a deficiency in the clinic's process. In solving this problem, reminders are implemented to assist staff on consistent documentation. Before implementing the project, microsystem analysis is conducted, and literature reviews are used to support evidence in using reminders to improve processes. A small test of change is performed to check improvement and found a slight increase in compliance. To further evaluate the process, generating new reports and reexamining charts will continuously perform. However, the limitation of this project is identified due to limited time. Finally, the need for this project is relevant to nursing because keeping track of data is necessary to sustain the needs of the patients. Furthermore, the project will lead to clinic’s acquiring monetary incentives to continue providing a high-quality of care.</p>
