Improving Patient Engagement Through Nurse-Led Blood Pressure Education and Interdisciplinary Collaboration

<p>The purpose of this project is to increase patient engagement and self-efficacy through patient-centered blood pressure education and interdisciplinary collaboration. By January 2017, 95% of outpatient registered nurses will develop efficient communication and interdisciplinary collaboration skills for patient-centered blood pressure teaching. There will be a designated nurse-led blood pressure education program in collaboration with the facility registered dietician, weight management, and dietary programs. The project will commence with the use of a RED TAG which serves as a reminder for Medical Assistants to recheck all elevated blood pressures prior to patient checkout at the end of their visits. Physicians will also be prompted to initiate patient education and engagement through the use of the RED TAG. Introduction of the RED TAG is an eye-catching tool that creates consistency in blood pressure rechecks.</p>
