Home Health: The Process of Assigning an Appropriately Trained Skilled Nurse to Provide Safe Patient Care

<p>The objective of this CNL project was to improve the process of placing an appropriately trained nurse in the client's home to provide safe, quality care. This project is relevant to nursing because of nursing accountability, patient safety, and lack of education of home health nurses. The population included home health patients in the Bay Area of San Francisco of all ages. The project had its base at a home health agency office in Emeryville, California with no actual hands-on patient contact. A CNL was added to the team to ensure appropriate training of the skilled nurse has been completed before the skilled nurse cares for the patient. The new process showed an increase in shared accountability between recruiters and clinical team as evidenced by full and timely participation of recruiters in starting and completing a form before care was provided. Additionally the new process ensured correct training of the skilled nurse as evidenced by 100% of the electronic assignment verification forms (ECAV) being closed before start of care with the particular nurse. The data used for this improvement project included tracking the ECAV forms from 5/28/15 to 7/29/15. Materials used were patient specific orientation forms, a process guide for recruiters and clinicians, and the ECAV form. The new process ensures safe care is provided by matching nurse competencies and skills validation to the patient.</p>
