Relationship Between Senior Clinical Competency Exam, Resident Readiness Survey, and ACGME Milestones

Background: The adoption of Competency-Based Medical Education (CBME) in U.S. medical education is complicated by the transition between undergraduate medical education (UME) and graduate medical education (GME). Predicting students' success as they move from UME to GME remains challenging, as there is currently no continuous assessment framework in place. Clinical competency exams predate current GME assessment frameworks, yet it remains unclear whether they are predictive of future performance. Methods: We analyzed performance on the summative clinical competency exam (SCCX) for students at our institution who matched into three specialties (Family Medicine [FM], Emergency Medicine [EM], and Internal Medicine [IM]) over a two-year period and compared these results to mapped ACGME milestone assessments and the AAMC Resident Readiness Survey (RRS). A linear regression model was created to determine whether any relationship existed. Results: Fifty-three students were included in the analysis (20 in EM, 17 in FM, and 16 in IM). Only a weak association was found between SCCX performance and 4 of 8 Family Medicine milestones. For all assessed milestones in EM (9) and IM (11), as well as the remaining 4 in FM, no association existed. No association existed between the SCCX exam and the RRS. Discussion and Conclusion: Despite long-term use and validity evidence for clinical competency examinations, the performance on our institutional SCCX is not strongly predictive of early residency performance as measured by ACGME milestones and the RRS.
