Perfil pro-inflamatorio / anti-inflamatorio en pacientes con síndrome coronario agudo y su relación con el pronóstico
Abstract
[EN]The pro-inflammatory state is an important condition for genesis of acute coronary syndrome (ACS) and how it relates to the destabilization of atherosclerotic plaque rupture and release pro-coagulant substance to be determined occlusive phenomena causing acute myocardial ischemia. However, the final result atherothrombotic process would be determined not only by the inflammatory state, but also by the level of anti-inflammatory components that may be present. Therefore, the hypothesis of this paper proposes that high concentrations of anti-inflammatory substances may influence the clinical course of ACS. To carry out the study, were quantified concentrations of two cytokines, interleukin-6 (IL-6) as a biomarker inflammatory interleukin-10 (IL-10) as anti-inflammatory biomarker in admission of patients with ACS, and are associated with hospital and postdischarge outcomes, considering episodes important as cardiac death, recurrent ischemic events, the need for revascularization and the occurrence of heart failure. Among most notable results include: 1) patients with SCA with a clear immune activation manifested by high concentrations of the cytokines studied, with a predominance of pro-inflammatory profile of the anti-inflammatory, 2) concentrations high IL-6 and IL-10 in patients with ACS are associated positively with 3 different risk markers for adverse outcomes, 3) IL-10 appears to be a stabilizing factor in the atherosclerotic plaque after an ACS, a situation evidenced by the reduction in the risk of recurrent ischemic events, 4) pro-inflamatorio/anti-inflamatorio profile is an independent predictor of the onset of heart failure during hospitalization for ACS, and 5) Concentrations of IL-6 and IL-10 increases the risk higher crude hospital mortality in patients with ACS
