Relación del índice arteriovenoso de la retina con las lesiones orgánicas subclínicas en pacientes hipertensos y diabéticos

Abstract

[EN] Objectives: Develop a Calculator AVIndex?? Software to measure the diameter of the vessels of the retina (DVR) and calculate the retina arteriovenous index (BTI), validate evaluating its reliability and validity for measuring DVR and BTI, and analyze the relationship of the DVR and AVG with blood pressure (BP), subclinical organ damage (LOS) and cardiovascular risk (CVR) in three groups of patients: diabetes, hypertension and without these comorbidities. Methods: Cross-sectional study in 402 patients. Measurement of DVR and IAV by semi AVIndex Calculator software, applied to retinal obtained with non-mydriatic digital retinal camera. Measurement of PA: Clinical and ambulatory. LOS Rating: Vascular: Carotid intima-media thickness, pulse wave velocity, ankle-brachial index (ABI) Cardiac: product-Cornell voltage duration (PDV), and Renal: serum creatinine, albumin-creatinine ratio (IAC ), glomerular filtration rate (GFR). Estimated RCV: Hypertension European Society criteria and D'Agostino. Results: Average age: 57.82 ± 10.70 years. Males: 60.9%. Reliability: interobserver intraclass correlation coefficient: 0.956 0.988?, Intraobserver: 0.969? 0.998. Validity: the IAC and retinal venous diameter are independent predictors of cardiovascular risk factors and renal LOS for each 0.1 um BTI decrease CVR increases 1.3%, and the IAC 2.73 mg / g. The study of correlations and multivariate analysis showed an association between IAV, DVR and PA, greater with ambulatory diastolic BP. Heart rate? Find correlation of retinal vein diameter. We found associations between IAV, DVR and LOS as ITB, PDV, IAC and GFR greater in diabetics. Association of retinal vein diameter with RCV is greater than the arterial diameter. Conclusions: The semi AVIndex Calculator software has high reliability and validity. There BTI Association and DVR with PA, the LOS and the CVR.
