Análisis de la supervivencia de las mujeres con cáncer de mama precoz diagnosticadas en el complejo hospitalario de Orense. ¿la determinacion inmunohistoquimica de ki-67, bcl-2 y p53 en el tejido tumoral aporta informacion pronostica en estas pacientes?

[EN]Breast cancer is the second most incident tumor in Europe. Two developments have stimulated interest in the investigation of prognostic factors in breast cancer. On the one hand treatments have increasingly effective chemotherapy for adjuvant treatment of women with breast cancer and side effects better know the medium and long term effects of these treatments. Furthermore screening programs implemented have allowed us to diagnose tumors at an earlier stage, smaller tumors without axillary lymph node metastases. Having accurate prognostic information will help us to avoid under-or over-treatment of our patients. In this paper we analyze the prognostic value of the immunohistochemical determination of Ki-67 protein, Bcl-2 and p53 in tumor cells of patients with early breast cancer. We included 643 women with breast cancer from 1997 to 2004. The expression of Ki-67 was defined as high at 35% of the samples, Bcl-2 was considered as positive in 68% of samples, and p53 was considered positive in 15.7% of cases. Median follow-up was 89 months. The median time to progression of our patients was 134.9 months (95% CI 130.56 to 139.26). In multivariate analysis, the variables retained significance were age (under 40 years), tumor size, axillary nodal status (more than 4 nodes), high expression of Ki-67. The mean specific survival was 144.27 months (95% CI 140.73 to 147.80).
