DNP project : improving cardiovascular health knowledge in a mid-Michigan community

Problem Cardiovascular disease (CVD) remains the leading cause of morbidity and mortality in the United States, yet many adults lack the knowledge and confidence needed to adopt heart-healthy behaviors. Limited access to community-based education contributes to gaps in health literacy, prevention, and early risk reduction strategies. Purpose The purpose of this project was to evaluate whether a community-based educational program delivered in a library setting improves cardiovascular health knowledge and self-efficacy among adults. Methods A program evaluation design delivered three educational sessions using the Embracing Aging curriculum developed by Kansas State University and approved by MSU Extension, including Eating Smart, Physical Activity, and Stress Management. Adult participants completed anonymous pre- and post-intervention surveys assessing knowledge, awareness, and confidence related to cardiovascular health behaviors. Descriptive statistics compared pre- and post-intervention scores, and qualitative responses were reviewed to assess participant perceptions. Results A total of 16 matched pre- and post-intervention surveys were analyzed. Mean total scores increased from 13.19 pre-intervention to 18.00 post-intervention, representing an average increase of 4.81 points. Improvements were observed across all three sessions, including Eating Smart, Physical Activity, and Stress Management. Participants reported increased confidence and identified interactive components as helpful. However, the small sample size (N= 5) limits the generalizability of findings. Conclusions Findings suggest the effectiveness of community-based cardiovascular education. This type of program can enhance knowledge and self-efficacy. It highlights the role of community partnerships in expanding access to preventive health education for diverse populations.
