Η τρίωρη μεταγευματική pH-μετρία του οισοφάγου στην κλινική πρακτική

Aim of this study was the investigation of the role of the 3-hour postprantial pHmetry (3hour pHmetry) in the assessment of: a) Gastroesophageal reflux disease (GOR) and its differentiation to the normal reflux phenomenon, and b) Post-therapeutic and/or post-operative follow-up of our patients. Sixty-five out of 82 patients (38 male; mean age 48 years and 27 female; mean age 51 years) presented with clinical or subclinical GOR symptoms without any previous operation and/or therapy. Seventeen patients (10 male; mean age 56 years and 7 female; mean age 59 years) had a previous history of antireflux procedure. Apart from 3hour pHmetry the study of our patients was based on: • A detailed history according to special protocol for the grading of the severity of symptoms, • A barium meal with follow-through of the oesophagus, stomach and duodenum in order to reveal: - Sliding hiatal hernia - Ulceration or oesophageal stenosis due to oesophagitis - Peptic ulcer of the stomach or duodenum combined with delayed gastric emptying or barium reflux in Trentelenburg's position. • Endoscopy of upper Gl tract for diagnosis of oesophagitis or ulceration, and biopsy. The above combination of procedures in the study of our patients was easy in its application, well-tolerated from the patient and diagnostically effective in every day practice of this complicated phenomenon. The results of this study showed that a proper conservative management of GOR may be effective in relieving the distressing symptoms of patients making obsolete operative procedures in most of them. In conclusione 3hour pHmetry proved to be an easy and usefull procedure in the diagnosis of the GOR and assessment of the effectivness of its treatment as well.
