Abstract
dc:descriptionThis study was designed to investigate the effect of dietary carbohydrate-protein relationship on the respiratory Function of 35 patients with chronic obstructive pulmonary disease (COPD) and hypercarnia. For this purpose in a 27 patients group, we supplied two specific diets, about 2500 kcal each, in two phases. Phase A diet, consisted of a high carbohydrate intake (375 gr of carbohydrate and 66.3 gr of protein) in contrast with the high protein administration of phase Β (137.5 gr of protein and 250 gr of carbohydrate). Each phase lasted one month and there was a 10 days interval with a free diet between them. Before the beginning of each phase and on the 7th, 15th and 30th day assessment of the following parameters was performed: blood gases and acid-base equilibrium, spirometric indices FVC, FEV₁, FEV₀․₇₅, MMFR and MBC, blood urea, creatinine, potassium, sodium, calcium, phosphorus, total serum proteins and albumins as well as 24 hr urine urea. In a 8 patients group, a reverse dietary administration, without a free diet interval, was attempted. This group was reevaluated on the 30th day and the investigation also included an assessment of the maximal inspiratory and expiratory mouth pressures (Pmo). The following are the most significant findings resulted from our study: Phase A. a) There was a small progressive increase in PaCO₂ and HCO₃ values, with a decrease in PaO₂ values. However, these changes were not statistically significant. pH remained unchanged. b) Mean values of spirometric indices and especially those expressing flows, showed progressive decrease which was statistically significant on the 30th day. c) There was no significant change in the various biochemical parameters. Phase Β. a) PaCO₂ and HCO₃ were progressively decreased while PaO₂ was increased and these changes were statistically significant on the 30th day. pH values remained unchanged. b) We noticed a significant increase in the spirometric indices values in the 15th and 30th day of the study. c) Serum total proteins and albumins, serum calcium as well as urine ureas were increased significantly at the end of the study. There was no deterioration of the renal function. In addition, in the 8 patients group, after a high protein administration, we noticed an important increase in maximal inspiratory and expiratory Pmo at the end of the 30 days period, comparing with the basic values. We also observed a statistically significant increase in spirometric indices, while PaCO₂ was significant decreased. In the same group, after a high carbohydrate administration we noticed a not statistically significant decrease in Pmo and spirometric indices, while PaCO₂ was not significantly increased, comparing with the after protein values. In conclusion our investigation showed that patients with COPD and hypercapnia may have significant improvement of their respiratory function and their blood gases after receiving a high protein nutrition.
