Διεγερτικές δοκιμασίες της προλακτίνης σε γυναίκες με ενδομητρίωση πριν, κατά και έπειτα από τη χορήγηση danazol

Ten patients with endometriosis, which was confirmed by laparoscopy, were treated with an oral dose of 200 mg χ 3 danazol daily for 6 months. Clinical symptoms were alleviated and menses disappeared in response to the treatment. After cessation of the treatment, menstrual bleeding returned in 1-2 months. For evaluation of prolactin (PRL) secretion a TRH stimulation test and an insulin challenge test were carried out in the follicular phase before treatment, in the 6th month of treatment and after reappearance of the second menses. The same tests were also carried out, only once, in the follicular phase on ten normal women. Results were followed: 1. The basal PRL levels were not significantly different in patients with endometriosis (before, during and after danazol treatment) compared with controls. 2. PRL response to TRH was not significantly different between patients with endometriosis (before and after danazol treatment) and normal women. 3. PRL response to insulin-induced hypoglycemia was not adequate in all subjects and not significantly different in the two groups, which were studied. 4. Danazol treatment reduced significantly the PRL response to TRH. These results, support the view that hyperprolactinemia should not be considered as a cause of infertility in endometriosis. On the other hand, the significant reduction of PRL response to TRH, during danazol treatment, may be associated with the low oestrogen level induced by the danazol treatment. Finally, the inadequate PRL response to insulin-induced hypoglycemia in patients with endometriosis and normal women shows that this provocative test is not much effective for hypothalamic stimulation of PRL.
