Communication skills, Emotional labor and Burnou in intensive care unit nurses.

This descriptive correlation study examines the relationship among communication skills, emotional labor,and burn out of ICU nurses. The subjects were 165 nurses who were working in the ICU of a university hospital. We used GICC (Global Interpersonal Communication Competence Scale), modified by Hur (2003),for rating communication skills. To assess emotional labor, tools developed by Morris and Feldman (1996) and translated by Kim (1998) were used. The CBI(Copenhagen Burnout Inventory) developed by Kristen, Borritz, Villadsen and Christensen (2005) and translated by Ham(2011), was used to examine burn out. For statistical analysis, we uses SPSS 18.0 for Windows to calculate frequencies, percentages, means, standard deviations, and Pearson’s correlation coefficients, as well as perform t-tests, one-way ANOVAs and Scheffé post-hoctests. The results were as follows. 1. The mean communication skills score of ICU nurses was 3.37± 0.36 points (maximum =5). 2. The mean emotional labor score was 3.24 ± 0.53. For the sub-domains, mean scores were as follows: frequency of emotional expression = 3.56± 0.57;, emotional attention = 3.36± 0.57; and emotional dissonance=2.81±0.77. 3. The mean burn out scores was 2.98± 0.75. The mean for personal burn out was 3.13± 0.84, job-related burn out was 2.96± 0.73, and patient-related burn out was 2.85±0.9. 4. Burn out was negatively correlated with communication skills(r=-.207,p=.008) and positively correlated with emotional labor(r=.476, p<.001). 5. Regarding the general characteristics of ICU nurses, communication skills differed with respect to working department (t=2.99,p=0.03) 6. The general characteristics of the nurse in the intensive care unit was 31 years of age or age of the emotional labor(t=3.223, p=.042),the current period is more than 10 years working in the department (F = 3.13, p = .027), the nervous system work departmentsignificantdifference in the intensive care unit(F =3.74,p=.012). We found relationship between communication skills and burn out of ICU nurses and between emotional labor and burn out. Nurses with better communication skills showed less burn out, and those with higher emotional labor experienced more burn out. Because communication skills reduce burn out, education and training for enhancing communication skills is necessary for ICU nurses. In addition, developing an intervention to reduce emotional labor and thereby decrease burn out is important.
