In-vivo-Implantation der Mikrodiagonalpumpe II beim Kalb : Hämodynamik und Hämolyse

Due to the increasing number of patients suffering from end-stage heart failure and the decreasing number of heart transplants the search for alternative treatment strategies is intensified. Artificial blood pumps may be used to bridge patients to transplant or might even be implanted as destination therapy. Despite considerable improvements during the last years the rate of complications remains high in regard of hemodynamic and technical failures, wound healing disturbances, thromboembolic events and bleeding complications. Therefore the development of new blood pumps is strongly encouraged. This study examines the microdiagonal pump II, an intrathoracic implanted, non-pulsatile pump which was developed at the Institute for Biomedical Engineerings, Aachen. The pump was implanted in six calves weighing 80-90 kg for seven days. It was placed in the left thoracotomy via a left-sided thoracotomy. The inflow cannula was inserted into the left atrium and the outflow cannula was anastomosed with the descending aorta. Pump flow was adjusted to 2_3l/min. Blood samples were taken daily during the examination period. Pump data were recorded continuously. Transesophageal and transthoracic echocardiographies and hemodynamic measurements were performed before and after implantation as well as before explantation of the pump. At the end of the examination period the animals were sacrificed and the pump explanted. Afterwards the pump and the inner organs were examined micro- and macroscopically. Reduction of left ventricular cardiac output, left ventricular enddiastolic and endsystolic diameters and the ejection fraction proved a partial unloading of the heart by the MDP. The hemodynamic parameters such as mean arterial pressure, mean pulmonary artery pressure, pulmonary capillary wedge pressure and cenral venous pressure remained stable during the examination period. The pump flow was almost constant over the entire examination period with 3,1 l/min by 6000 revolutions per minute. The MDP showed only minor hemolytic activity as indicated by constant values for free plasma hemoglobin. A slight increase of hemoglobin and hematocrit values was detected postoperatively. These parameters normalized until the end of the experiments. The end organ function as determined by the blood parameters for total bilirubin and creatinine was not significantly impaired. After an initial increase these values normalized or even dropped below the preoperativ values. The temporary increase can be explained by short periods of low arterial blood pressure occurring during partial clamping of the descending aorta for the anastomosis with the outflow graft. The microdiagonal blood pump proved to be a reliable and effective assist device for partial unloading of the heart. Due to the promising data from these experiments long-term studies are now indicated.
