Knochenaugmentation im Oberkieferseitenzahnbereich mit unterschiedlichen Ersatzmaterialien und Induktoren : eine klinische histomorphometrische Studie

In the oral implantology, one requires a sufficient bone-offer to insertion of implants. Today, a dentist can create a sufficient bone-camp with different bone substitutes. The aim of the present study was to evaluate bone formation following maxillary sinus augmentation using tissue-engineered bone BioSeed®-Orale Bone and bovine bone substitute material Bio-Oss® in combination with Platelet Rich Plasma (PRP) or autogenic bone by means of histologic and histomorphometric examination of biopsies. 27 patients with an average age of 58.92 years received sinus augmentation according to the technique described by Tatum (1986). The patient maxillary sinus was replenished with BioSeed® Oral Bone and Bio-Oss®. After an average healing phase of 6.8 months, trephine burrs were used to take 61 bone biopsies from the augmented sinus region. Afterwards 74 implantats were inserted in augmented posterior maxilla. Histomorhpometric analysis of ground sections from the bone biopsies prepared according to the standard method of Donath and Breuner produced an average percentage of newly-formed bone of 23.03% ± 2.98% in the BioSeed-Oral Bone group, 20.36% ± 1.65% in the Bio-Oss® and PRP group and 21.97% ± 2.04% in the Bio-Oss® in combination with autogenic bone group. Histologically, the bone biopsies from the BioSeed®-Oral Bone patients revealed mineralized trabecular bone without remnants of biomaterial. Osteocytes were apparent within bone lacunae. In the Bio-Oss® in combination with PRP or autogenic bone groups histologically newly developed bone became evident, partly invaginating the particles of apatite and forming bridges in the form of trabeculae between the individual Bio-Oss® particles. In this group, the results clarify the osteoinductively qualities of the PRP on bone substitute. Furthermore, PRP clearly becomes transfer its osteoinductively qualities through the diverse newly-formed bone. In the Bio-Oss® and autogenous bone group forms the autogenic bone graft because of its osteoinductively and osteogenetic potency growth-center of newly-formed bone. A reabsorption has not been recogignized in the BioSeed®-Oral bone group. Despite the absence of osteoclastic activity, the inward growth of bone indicates slow resorption of the xenogenic bone graft material in the Bio-Oss® in combination with PRP or autogenous bone. When the implants were uncovered, after an average healing phase of 6 months, 3 of the 74 implants got loose. 2 of those got loose in the BioSeed®-Oral Bone Group. Thus survival rate was 90.90%. 1 implant got lost in the Bio-Oss® and PRP group. The survival rate was 96.15%. In the Bio-Oss® and autogenic Bone group the survival rate was 100%, because no implant gets loose.
