In a novel full-scale in-vitro recirculation dialysis model the influence of dialysis membranes (Cuprophan® and Polysulfone) on biocompatibility parameters was investigated. Thrombin-antithrombin complex (TAT), platelet loss, platelet factor 4 and C5a were measured to detect activation of the coagulation cascade, platelet activation and complement activation.The contact system is generally believed to be the main trigger of the coagulation cascade during extracorporeal circulation. However, the extent of contact activation, its role for intradialytic thrombin generation as well as the influence of different dialysis membranes have not been well established. The activation of the contact system was evaluated using a newly developed immunoassay for factor XIIa-C1-inhibitor complex.Results: With blood from healthy volunteers there was no difference in thrombogenicity between Cuprophan® and Polysulfone. Cuprophan® caused a much stronger complement activation than Polysulfone.Only in-vitro dialysis with the negatively charged dialysis membrane AN69 (polyacrylonitrile) led to a rapid increase of the factor XIIa-C1-inhibitor complex. Despite a significant TAT generation with the more neutral Cuprophan®- and Polysulfone-membrane the factor XIIa-C1-inhibitor complex remained unchanged.Conclusion: The in-vitro recirculation dialysis model is a reliable method to evaluate biocompatibilitiy of dialysis membranes.The represented data challenged the common assumption that activation of the contact system with generation of factor XIIa is the main trigger for coagulation and thrombus formation in hemodialysis. Only the negatively charged AN69 membrane strongly induced contact activation.
