Effekte der Co-Induktion mit Midazolam auf eine total intravenöse Anästhesie mit Propofol und Remifentanil zur Entfernung von drei oder vier Weisheitszähnen in Intubationsnarkose

Objective: Does co-induction and co-maintenance with midazolam during total intravenous anaesthesia (TIVA) with propofol and remifentanil reduce the propofol dosage by 50% without any decrease in quality of anaesthesia? Methods: 100 patients undergoing general anaesthesia for operative removal of three or four wisdom teeth were randomly divided into two groups (oM, mM). In the oM-group anaesthesia was started with a bolus of 1 µg/kg remifentanil in addition with 1-2 mg/kg propofol and was maintained with 0,5 µg/kg/min remifentanil in addition with 100 µg/kg/min propofol. In the mM-group patients received 1 µg/kg remifentanil with 0,5-1 mg/kg propofol for induction and 0,5 µg/kg/min remifentanil with 50 µg/kg/min propofol ongoing and additional 0,03 mg midazolam at the start of the anaesthesia and every 30 min perioperative. In the postanaesthesia care unit all patients received patient controlled analgesia (PCA) with piritramid. The intensity of pain was recorded with a numerical scale. The next day patients were answered postanaesthesia questionaire concerning complaints and feelings. Results: Both types of TIVA showed a good quality of anaesthesia and stability of haemodynamic parameters. The mM-group patients were extubated 1,54 min (6,73 min vs. 8,27 min) earlier and the dosage of propofol was reduced up to 59,56% for induction and 48,41% for maintenance. By this savings of 3,93 DM per patient were achieved. Postoperative there were no differences between the two groups in using PCA and answering the questionaire.
