Blau/Gelb-Perimetrie bei Patienten mit altersabhängiger Makulopathie

dc:descriptionShort wavelength automated perimetry in age related maculopathy Age related maculadegeneration (AMD) is the most common cause for legal blindness in industrial countries. Previous studies reported the predictive value of the short wavelength sensitive (SWS) cone mediated sensitivity for visual outcome in AMD. In this study SWS sensitivity was measured by commercial available blue on yellow perimetry in patients with non-exudative age related maculopathy (ARM) and compared with the presence of morphological risk factors and the status of the fellow eye. Morphological findings which were not seen ophthalmoscopically can be quantified with the scanning laser ophthalmoscope (SLO). Early changes can be visualized and digital quantified. The question to be answered was to see, if a morphological fundus appearences and the SWS cone mediated sensitivity could give a predictive value for functional loss. In a prospective cross sectional study 126 patients with ARM an good visual acuity were tested. Central visual acuity (blue on yellow) were obtained with conventional perimeter. Fundus slides were graded by two independent observers for soft drusen and presence of focal hyperpigmentation. Additional in 35 patients images with the SLO were created and morphological findings were graded digitally. Mean sensitivity and standard deviation of all patients exibited a significant reduction with age. Patients with soft drusen had significantly lower sensitivity than those without, whereas there were no differences in visual acuity. Sensitivity was also reduced in those eyes with fellow eyes has having a sight threatening complication of AMD. Digital evaluation of the images of the SLO show a high involvement of soft drusen in the area centered arount the fovea, followed by the field temporal to this. There was a high relationship of soft drusen an SWS cone mediated sensitivity. SWS sensitivity loss is associated with common risk factors for progression to AMD. Short wavelength automated perimetry is moderately rapid and readily available.
