Auswirkung eines ergänzenden Arzt-Patienten-Gespräches auf die Anpassung chirurgischer Intensivpatienten

The aim of this investigation was to find out, if an additional doctor-patient-conversation would influence the state anxiety and the postoperative adaptation of surgical intensive patients. For this purpose 37 patients – out of a random sample of N=77 patients – received a half-standardized doctor-patient-conversation in addition to the routine preoperative information that is usually given by the surgeon and the anaesthetist. The additional dialogue includes sensorial and general information about the intensive care unit. The patient’s preoperative stress was measured by the state anxiety. The state anxiety was recorded on the basis of a questionnaire that assesses cognitive, affective and somatic symptoms of anxiety (KASA). In order to measure the postoperative outcome, the patients received the KASA-questionnaire again after leaving the intensive care unit. Simultaneously, the subjective state of health was documented by emotional sensations and physical functions. Besides, the patients assessed the dimension of pain incurred after the operation. As a result of this investigation it has been demonstrated that the additional conversation between physician and patient induced a significant decrease of preoperative stress measured by the patient’s state anxiety. There was no effect regarding the postoperative adaptation. The results were discussed with respect to the questions formulated in the beginning and former research results. Finally some perspectives for future research work were presented.
