Wiederholungsrisiko und anamnestisches Risikoprofil bei hypertensiven Schwangerschaftserkrankungen (HES)

The objective of this epidemiologic study was the determination of a risk profil for the prerecognition of hypertensive pregnancies (HES) on the one hand and the determination of the risk for a HES in a following pregnancy on the other hand in the German population with the help of a standardized questionnaire. The structure of this study is based on ISSHP classification and separates this research work in the parts HELLP-syndrome (HELLP) (n=420), preeclampsia (PE) (n=367) and gestational hypertension (GH) (n=111). The nulliparity is a risk factor for all three kinds of HES whereas the age of the mother is an independent factor of the PE. A higher body mass index leads to an increased risk for the development of a PE or GH, but not for a HELLP. A positive family history (hypertension, dyslipoproteinaemia, myocardial infarction, stroke, diabetes) as well as clinical symptoms during pregnancy (headache, edema, upper abdominal pain, vertigo) show no significant dependency for an increased risk for all three diseases. The risk of a HES in a following pregnancy is close to 47,8% for patients after HELLP, 60,9% after PE and 79% after GH. Patients with HELLP have the highest risk (13,2%) to develop it again. The time periode between two pregnancies is not a risk faktor itself for a HES. Perinatal complications (preterm delivery, intrauterine growth restriction, neonatal death) occuring mostly in HELLP patients and decrease significantly in the following pregnancies in all hypertensive disorders.
