Prospektive Evaluation eines portablen Ultraschallgerätes in der Notfall- und Routinediagnostik einer internistischen Abteilung

dc:descriptionAim of the trial was to find out the reliability of diagnosis detected with an portable ultrasound device: the SonoSite 180 Plus. The mean question which should be answered was, wether the investigators would detect the clinical decisive pathological findings by using the SonoSite 180 Plus as often as by using an high-end ultrasound machine (HDI 5000, Philips/ATL). Therefore in 574 cases non selected patients were examined under identical standardized conditions with both machines for ten minutes each by the same investigator. Findings as the clinical decisive meandiagnosis and four additional clinical relevant diagnosis, the used technical features (SonoCT, THI, highfrequency 12 MHz linear array probe etc.) and imaging quality were documented directly after each examination. Results: The same diagnosis was found with both machines in 89% (99/111) of pleurasonographies, in 85% (252/297) of upper abdominalsonographies and 58% (96/166) of lower abdominal- and bowelsonographies. In 75% (138/185) of emergencysonographies there was a coincidence between both machines, when the sonography of the lower abdomen was excludet this rate rised to 89% (105/118). 49% (45/92) of all pathological findings in the lower abdominal region where detected with an 12 MHz linear array probe of the HDI 5000. In 139 examinations the imaging quality of the portable device was defined als poor, in 40 cases defined als verry good. In both groups there was the same rate of false negative diagnosis with SonoSite (17,5%). Conclusions: The Sonosite 180 Plus offers valide diagnosis if a larger ammount of free fluid in the pleura, larger structures of the upper abdomen or emergencies are aim of interest. The results of the examination of the lower abdomen respectively the bowel and of small structures in DHC an pancreas are not satisfying at this time. The mesured imaging quality is no praedictor of the validity of the Diagnosis.
