In-vivo-Untersuchungen zur Gewebeverträglichkeit einer neuartigen, leichtgewichtig-großporigen Mesh-Generation zur Narbenhernienreparation

The marked reduction in recurrence rates following reinforcement of the abdominal wall by Meshes has promoted their increasingly widespread use. The extent of the scar tissue is markedly regulated by the amount and structure of the incorporated material and is responsible for the rate of local wound complications. Correspondingly, minimization of the alloplastic implants should be favourable. In an explorative study with rats, the results after implantation of two lightweighted Meshes ( Prolene210® 25,8 g/m2 , Pronova201® 54,7 g/m2 ) with a pore-size of 3-4 mm on the one hand, and four constructions , which were combined with the resorbable Poly-e-caprolactone ( Mono-Prolene218/220® 49,6 / 55,3 g/m2 and Mono-Pronova211/217® 78,5 / 84,2 g/m2 ) with the same pore-size on the other hand, were compared. The mesh samples were investigated by light-microscopy as well as immunhistochemistry. The morphometric results confirmed an inflammatory proliferative foreign-body reaction within different cell-turnovers in the recipient tissues. These reactions are mainly influenced by the used mesh modifications. As result, the different Monocryl®-constructions showed a remarkable decrease in inflammation and fibrosis, as well as a significant reduction in apoptosis-rate, irrespective of the quantity of the applied Monocryl®. All investigated Meshes seem to be qualified for use on humans, whereby the advantages are to find on the side of the combined constructions with Monocryl®.
